Favorable Endorsement to CDA
  1. Letter of Request signed by the cooperative’s Chairperson
  2. Copy of Notarized Articles of Cooperation and by – laws;
    1. Include Specific Business Operation in Article VII of the Articles of Cooperation (from point of origin to point of destination for the proposed or existing operating complied with the minimum number of franchise
    2. Include Provision on Vehicle Operation in the By-Laws particularly on the following: Vehicle Ownership – All members of the cooperative shall transfer the management of their units to the cooperative by means of Management Agreement for purpose of franchise consolidation in order to provide efficient transport service including but not limited to the designation of routes, scheduling of trips, registration of vehicles and overall supervision of unit. The cooperative shall procure transportation vehicles which shall be registered in the name of the cooperative which shall be responsible for the operational expenses of the units including maintenance, registration, fuel and driver’s salaries and operators’ benefits.
  3. Copy of Simplified Economic Survey (CDA)
Accreditation (After Registration to CDA) For Franchise Holders Only
  1. Letter of Intent signed by the cooperative’s Chairperson
  2. Photocopy of Certificate of Registration issued by the Cooperative Development Authority (CDA)
  3. At least 15 OR/CR together with their copy of franchise with the same type of service and route
  4. Bank Certification of Deposit representing the paid-up capital of the cooperative that will operate the following:
    5. PUJs and Multi-cabs Php 300,000
    Airconditioned Vans, Taxis, or TNVS Php 750,000
    PUBs and Minibus Php 1 Million
    Trucks for Hire (2 units) Php 200,000
    Tricycles Php 15,000
Accreditation (After Registration to CDA) For NON-Franchise Holders Only
  1. Letter of Intent signed by the cooperative’s Chairperson
  2. Photocopy of Certificate of Registration issued by the Cooperative Development Authority (CDA)
  3. Bank Certification of Deposit representing the paid-up capital of the cooperative that will operate the following:
    4. PUJs and Multi-cabs Php 300,000
    Airconditioned Vans, Taxis, or TNVS Php 750,000
    PUBs and Minibus Php 1 Million
    Trucks for Hire (2 units) Php 200,000
    Tricycles Php 15,000
Certificate of Good Standing (CGS)
  • Letter Request
  • Duly accomplished OTC Annual Report – Template
  • Certificate of Compliance (COC) from CDA
Authorized Representatives to Transact business with LTO/LTFRB and other concerned Agencies
  • Letter request duly signed by the Chairman
  • Approved Board Resolution Designating the Authorized Representative to Transact Business in behalf of the Cooperative
